Commercial Plans
Commercial HMO
- 1. Call the members services number on your insurance card.
- 2. Tell the insurance representative which doctor you want to see.
- 3. You will be mailed a new insurance card within a week or two.
- 4. Call our office and book an appointment.
Commercial PPO
- 1. Choose the doctor you want to see.
- 2. Call that office to schedule an appointment.
