What is a CIF sports physical?

A sports physical is a preparticipation physical evaluation. The American Academy of Pediatrics describes it as the visit that decides whether an athlete is medically eligible to practice and compete. CIF State posts the standard packet: history form, physical exam form, and medical eligibility form.

The visit is built around heart, lungs, joints, concussion history, asthma, medications, and anything that could make a sport unsafe. It is not a full well-child checkup unless you book both on purpose.

Local high schools in Temecula Valley Unified still send families to athletic clearance portals and a doctor physical plus history form. Great Oak Athletics links an athletic clearance instruction page and a doctor physical and history form. Chaparral already lists 26/27 athletic clearance on its homepage. Your school’s packet wins if it differs from the CIF sample.

AAP guidance: complete the PPE in the medical home when you can, and aim for at least six weeks before the first preseason practice so there is time if the clinician needs a follow-up. Ask the athletic director how long last year’s clearance remains valid.

How is a sports physical different from a school physical?

Parents use “school physical” for three different things:

1 CIF / club PPE for athletics. This is the clearance signature.

2 Well-child or annual checkup with growth and development. Useful, not what the coach stamps.

3 Immunization record for enrollment. California public schools check shots. They do not, as a statewide CDPH rule, require a separate head-to-toe physical for every TK–12 enrollment.

CDPH Shots for School is the shot list. If the office only needs vaccines, say that when you book so the visit is coded correctly.

A sports physical does not automatically update every school shot. A well visit does not automatically produce a signed CIF eligibility form unless the clinician completes that paperwork. Tell the scheduler: “CIF sports physical, school’s form in hand,” or “shot record for TK / 7th grade,” or both.

What forms should I bring?

Bring the school’s current packet, not last year’s PDF from a group chat.

• CIF or district history form, filled out by parent and athlete before you sit down (CIF history in English and Spanish)
• Physical examination and medical eligibility pages for the clinician to sign
• Athletic clearance account login if the school uploads a scan (TVUSD schools often do)
• Shot record or CAIR printout if you also need school enrollment
• Glasses, inhaler, ADHD meds list, and specialist letters (cardiology, ortho)

AAP asks for a private history review, including a moment with the athlete alone when that is appropriate. Gym or locker-room mills are a poor setting for that conversation.

If the athlete has a disability, AAP publishes a supplemental history. Special Olympics uses its own medical form. Do not swap those packets.

What shots does California actually require for school?

CDPH TK–12 requirements (parents show the immunization record):

New TK/K–12 admissions and transfers

• DTaP / DTP / Tdap / Td: 5 doses (fewer in specific age/timing cases CDPH lists)
• Polio: 4 doses (3 if one was on or after the 4th birthday; oral polio on or after April 1, 2016 does not count)
• Hepatitis B: 3 doses (required at admission to any grade except 7th)
• MMR: 2 doses, both on or after the 1st birthday
• Varicella: 2 doses

Students advancing to 7th grade

• Tdap: 1 dose (whooping cough booster, usually at 11+)

RFMG can review the chart, give due vaccines when appropriate, and print what the school nurse will accept. Medical exemptions in California go through the state’s process, not a sticky note on a PPE form.

When should we schedule so we are not late for tryouts?

Winter sports (basketball, soccer, wrestling, water polo) stack clearances in fall. Spring sports follow. Do not wait until the week practice starts.

Book six weeks out when you can. If the clinician hears a murmur, sees high blood pressure, or needs an old injury cleared, the athletic office will not take an unsigned form.

Same-week visits happen when panels are open, but peak August and January fill fast. Use MyChart for real openings across 13 clinics, not only the office closest to campus.

Can urgent care sign the form?

Many districts accept a signed PPE from a qualified MD, DO, NP, or PA. AAP still prefers the primary care medical home, because the chart has asthma plans, heart history, and mental health context that a one-off sports mill will miss.

iCare Urgent Care is for illness and injury that cannot wait, not a substitute for a planned PPE. iCare is not 24 hours. For a sports physical, book family medicine or pediatrics during regular clinic hours.

If your child is already sick (fever, sprain, concussion follow-up), treat that first. A PPE on a sick day is a wasted signature.

RFMG accepts most commercial PPO and HMO plans. Confirm your plan when you schedule. Bring the school form. A signed eligibility page is the product the coach needs.
